Pan Haven Community Day Program is a Florida APD iBudget Waiver provider serving adults with developmental disabilities in Clay County, Duval County, and surrounding Northeast Florida communities.
We believe every adult — regardless of ability — deserves a rich, fulfilling day that goes beyond the walls of home. Our community-based companion services are built entirely around each person's unique goals, interests, and vision for their own life.
We partner closely with participants, their families, and Waiver Support Coordinators to design individualized plans that make real progress toward ISP goals while keeping life joyful and engaging.

Eligibility & Who Can Join
the Pan Haven Community
Pan Haven serves adults enrolled in the Florida APD iBudget Waiver with Level 1 Companion Services authorized in their Individual Support Plan across Clay County, Duval County, St. Johns County, Nassau County, and surrounding Northeast Florida communities.
Autism Spectrum Disorder
Adults with ASD finding meaningful community connection
Intellectual Disabilities
Individualized support for adults with intellectual disabilities
Down Syndrome
Community inclusion and skill building for adults with Down syndrome
Cerebral Palsy
Accessible community services for adults with cerebral palsy
Other APD Diagnoses
Prader-Willi syndrome, spina bifida, and other qualifying conditions

Age 21+, APD iBudget enrolled, Level 1 authorized in current ISP

How Enrollment Works
Initial Contact
Call or email us to discuss the participant's goals, needs, and Level 1 authorization.
WSC Coordination
We connect with the participant's Waiver Support Coordinator to obtain the current ISP and authorization.
Enrollment & Planning
Complete enrollment paperwork and develop the Agency-Level Service Plan with the participant and family.
Staff Match & Welcome
We match the participant with a companion and schedule a welcome orientation before services begin.
Services Begin
Community outings start on the agreed schedule, with ongoing communication with family and WSC throughout.
