Pan Haven Community Day Program is a Florida APD iBudget Waiver provider serving adults with developmental disabilities in Clay County, Duval County, and surrounding Northeast Florida communities.

We believe every adult — regardless of ability — deserves a rich, fulfilling day that goes beyond the walls of home. Our community-based companion services are built entirely around each person's unique goals, interests, and vision for their own life.

We partner closely with participants, their families, and Waiver Support Coordinators to design individualized plans that make real progress toward ISP goals while keeping life joyful and engaging.

How Enrollment Works

Initial Contact

Call or email us to discuss the participant's goals, needs, and Level 1 authorization.

WSC Coordination

We connect with the participant's Waiver Support Coordinator to obtain the current ISP and authorization.

Enrollment & Planning

Complete enrollment paperwork and develop the Agency-Level Service Plan with the participant and family.

Staff Match & Welcome

We match the participant with a companion and schedule a welcome orientation before services begin.

5

Services Begin

Community outings start on the agreed schedule, with ongoing communication with family and WSC throughout.
